Community Support Services (CSS South)

Community Support Services South (CSS) – a specialist service within Ember – is responsible for engaging and supporting people who face challenges with their mental wellbeing. The aim is that everyone can engage in their chosen community and achieve their fullest potential. This includes working with the individual as well as a commitment to work actively in partnership with family/whanau, Clinical Partners and/or GP services. Our catchment area is aligned with our CMDHB Clinical Partners Rapua te Ao Waiora and Nga Raukohekohe(Papakura and Franklin). The employees act as a positive role model throughout the process.
The service promotes self management and self reliance and supports people with accommodation, transport, training, employment, self care, and personal and interpersonal skills so that people are enabled to live independently in the community of their choice. Staff work hard to guide and mentor using each and every opportunity to create independence, walking alongside an individual with the goal of total independence.
The service is for people who are currently, or recently been, engaged with CMDHB Clinical Partners who are finding it difficult to live in their community, or to reach their desired goals.
As a service we have fully embraced the Strengths Model and Te Whare Tapa Wha.

Community Support Services (CSS Central)

The Community Support Services Central – a specialist service within Ember – is responsible for engaging and supporting people who face challenges with their mental wellbeing. The aim is that all people can engage in their chosen community and achieve their fullest potential. This includes working with the individual as well as a commitment to work actively in partnership with family/ whānau, Community Mental Health Centres and General Practitioners.
The service promotes self-management and self-reliance for all in areas such as accommodation, transport, employment, self-care, personal and interpersonal skills to enable people to live as independently as they are able to in the community of their choice.
Our geographical area covers people living in Central Auckland, Waiheke Island and West Auckland. The service is for people who face low to severe mental health challenges who are finding it difficult to live in their community, or to reach their desired goals.
Our Registered Health Professionals/ Social Workers and Support Workers have a specialised and a wide range of skills applicable to the respective qualification, which include knowledge in communication skills, assessments, planning and implementation, psychiatric medication (including side effects) and a good knowledge of mental health disorders.
As a service we have fully embraced the Strengths Model and therefore all work done with individuals is based on their strengths and celebrating achievements

Community Support Team
Our Community Support Service consists of 3 mobile teams supporting people in their recovery from mental health issues by offering a wide range of practical and empowering services.
In Waitemata, we also provide Packages of Care to those who require higher levels of flexible and responsive support. These care packages are designed to navigate and coordinate a way through complex and challenging situations. Additionally, we also have a multi-lingual, culturally sensitive Asian Support team, who work with those whose English language skills are limited to help them feel part of the community and explain what services and agencies are available.
In Central Auckland, we also provide services to meet the needs of families/whānau where a parent experiences mental health or addictions issues. Working with children and adolescents, this often necessitates a different approach.

Beachhaven (Oruamo)
Beach Haven is a packages of care community service for those clients who have 4 hours or more a week, support packages. At Beach Haven whaiora can effectively work on practical, emotional and social skills that will help them live a fulfilling life within the community.
Beach Haven is for those who would benefit from high levels of complex support and are under care from a community mental health team. The service provides longer-term support.
Awhi Ora

The Awhi Ora – Supporting Wellbeing Team (Previously known as The Tamaki Mental Health and Wellbeing initiative) was launched in 2013 to help create a new experience of mental health and wellbeing support in Tamaki. This was being achieved through working together with the local community, social agencies, local providers, NGO’s and GP practices.
Awhi Ora is a part of our Community Support Services Central, a specialist service within Ember. It is committed to providing support to clients, by helping to find resources in their community to be able to live their lives to the fullest. This includes working with the client along side of family/whanau, Community Mental Health Centre’s and General Practitioners if needed.
The service is for people who are currently experiencing personal/ practical challenges and are finding it difficult to perform day to day tasks or to reach their desired goals.
The service promotes self management and independence for clients with supports such as accommodation, transport, employment, personal and interpersonal skills, budgeting, education, WINZ advocacy and self-care; so that it will enable them to live well in the community of their choice. Our clients live in Central Auckland.
The Occupational Therapist/ Social Worker and Support Workers have a wide range of specialised skills applicable to their respective qualification, which include communication skills, assessments, planning and implementation, as well as a good knowledge of mental health and well-being. The employees act as a positive role model throughout the process.

Community Living Service - Achieving Potential

The Community Living Service (CLS) – a specialist service within Ember – is responsible for engaging and supporting individuals with a mental illness, who have high and complex needs. The aim is to engage people into their chosen community which will enable them to achieve to their fullest potential.
Ember’s CLS service – based in premises at Papakura and Pukekohe – has a geographic spread complementing the area of services provided by their CMDHB clinical partners, Rapua te Ao Waiora and Nga Raukohekohe(Papakura and Franklin). Reporting from, and to, the clinical team takes place on a regular basis.
Although a need for clinical awareness is important; it is the role of the clinical teams to design an individual’s clinical care. The Community Living Specialist is however responsible to observe any changes in the individuals well-being and is responsible to report these to the clinical team.
The CLS team actively engages with the local community, focus on education, and building supportive relationships with various local facilities such as local businesses, churches and training centres.
Referral to this service is via the clinical teams. The service focuses on such areas as chosen housing options, ability for people to access and utilize transport, daily living skills; and a huge focus on developing community supports for individuals to use during times of mental unwellness and stress. CLS achieves this through a holistic approach and will cover such supports as family, friends, clinical teams, churches and support groups.
Community living specialists have a good knowledge in communication skills, assessments, planning and implementation and a good knowledge of mental health disorders.

Franklin Integrated Locality Care Team (ILoC)
Mental health and addiction support for primary care
Based in Franklin. Our team of mental health and addiction clinicians combines with support workers to engage with primary care professionals and people under their care. We provide support to primary care by liaison, advice and brief intervention in primary care settings. We are a complement to the existing range of specialist community mental health services.
